Agreement to these terms

These Terms of Service govern your use of replacestatefarminsurance.com (the "Site"), which is operated by The Harbor Group USA, Inc. ("Harbor Group," "we," "us," or "our"). By using the Site, you agree to these terms. If you do not agree, do not use the Site.

Informational purpose only

The Site provides general information about medical plan options and a way to request a coverage review. Site content is not an offer of coverage, an insurance contract, a quote, medical advice, legal advice, or tax advice. Submitting an inquiry does not create coverage or guarantee that a plan is available or appropriate for you.

Plan availability, network access, pricing, benefits, eligibility, and underwriting depend on state, program availability, and individual case details. Final terms are controlled by the applicable plan and enrollment documents, not by this Site. You should review those documents and ask questions before making a decision.

Independent brokerage

Harbor Group USA is an independent brokerage. It is not affiliated with, endorsed by, or sponsored by State Farm. State Farm is a trademark of its respective owner and is referenced only to identify the audience affected by a coverage change.

References to provider networks identify network access only and do not mean that a network company insures or sponsors a plan.

Your communications with us

You agree that information you provide will be accurate to the best of your knowledge and that you have authority to provide it. Do not submit medical records, payment-card details, passwords, or other unnecessary sensitive information through Site email links.

The coverage form opens your own email application with a draft message. You decide whether to send it. Email delivery is not guaranteed, so contact us again if you do not receive a response.

Acceptable use

You may use the Site only for lawful purposes. You may not:

• interfere with the Site's operation or security;
• attempt to gain unauthorized access to systems or data;
• introduce malicious code or use automated means that unreasonably burden the Site;
• impersonate another person or misrepresent your affiliation; or
